It is our pleasure to welcome you to Southern Crescent TBI. Exceeding your rehabilitation needs is our goal. We're committed to helping you and your family take the next step in your recovery. 


Admitting to our facility early in the morning or afternoon is the ideal time. This allows you to have time to meet the staff and become oriented to our facility. Your transportation arrangements should be discussed with your discharge planner/social worker. 

What to expect

After you arrive to your room, a nurse will conduct a detailed assessment of your medical and rehabilitation needs. You will meet different members of the care team. Your treatment plan including your specific goals will be discussed. 

What to Bring

Please bring comfortable clothes and rubber closed toe shoes for therapy.

Daily Routines

To help reach your goals quickly, participation in therapy daily is essential. We encourage you to prepare for your individualized therapy sessions in the morning. Nurse aids and the therapy team are available to assist you with ADL's. 

Therapy Sessions

Therapy sessions are scheduled three hours per day, five days per week. This includes relearning everyday skills such as dressing, eating, and grooming. There will also be sessions in which you will participate in therapy in our rehabilitation gym. 

Discharge Planning

Discharge planning begins at admission. Our discharge planner coordinates your care and works with you and your family, the treatment team and community agencies to ensure that you are prepared to transition back home or into the community,  


Our nutritionist and registered dietitian create meals and snacks that contribute to a healthy diet. 


No overnight stays
Monday-Friday  3PM-7PM
Saturday- Sunday  8AM-7PM
